I have read and agree to these terms and conditions described here in the Service Agreement: We receive this request for Service with the true understanding that it is for legal and lawful purposes and will not be held responsible for any and all damages, claims and/or legal actions that may result in carrying out this request. Furthermore, we do not guarantee service will be successful. Once a request has been made, service is deemed in effect and therefore billable. We only provide services once payment arrangements have been made. We accept all forms of payment: CC, Check, Bank Transfer, PayPal, and Cash. Payment is due upon receipt of our Invoice. After 30 days from the date of invoice, there is a finance charge assessed at 18% APR, 1.5% per month, on the unpaid balance. There are NO REFUNDS, and we reserve the right to refuse service to anyone for any lawful reason. By signing this online Request for Service, you acknowledge that you understand this service agreement and are confirming that you have read our privacy policy that is linked below.

Authorization of Credit Terms*

I have read and agree to these terms and conditions described here in the Credit Card Authorization: We agree to provide the very best service possible. In return, we appreciate and expect your willingness to trust us with your card information. Once you click submit, your request is sent to our office and the Credit Card details are hidden from view. The card information is stored in our secure payment portal (stripe) for your convenience and peace of mind. By Submitting this request you are confirming that you understand that the charges made for services rendered are non-refundable and non-contestable. This authorization agreement shall act as a guarantee of payment for any and all charges incurred for services rendered. We reserve the right to charge your card for any past due balance not paid within our credit terms. Because these transactions are transmitted electronically, it is not required that we keep a physical credit card on file for a charge on the card to be valid. We send out statements on the first of each month for unpaid balances. If payment is not received by the 15th of that same month, we charge the card on file for the total "past due" balance. If you are sending a check too close to the due date, please give us a call and we can discuss alternative payment options.
