I, the undersigned and hereby agree to employ the owner of MINTZ PRIVATE INVESTIGATIONS, a licensed and Insured private investigator to conduct my investigation which is confidential, and agree to hold the same confidence except for legally permissible reasons in compliance with state, local, and federal laws and regulations. The information provided will not be used to cause emotional or physical harm to the subject and I agree not to use the information for any criminal act, harassing, or stalking. As the client, we assume any & all personal responsibility for any violation of the provisions or any damage arising from my actions. The goal of this investigation is to ascertain the true facts of the case and the specific details of this investigation: litigation, marital investigation, trial pre-preparation, missing person, etc. I agree MINTZ PRIVATE INVESTIGATIONS and the owner will not assume any responsibility for any facts discovered during the investigation, now or in the future, and will pay for any reasonable attorneys fees. I understand Private Detective work and investigative work are not magic, it is understood that the investigator shall use his best efforts to investigate this matter set forth. I understand the results cannot be guaranteed as defined in the terms and conditions. MINTZ PRIVATE INVESTIGATIONS retains the right to provide or not to provide you with personal information obtained during the course of the investigation and will be handled on a one-on-one basis using our discretion. I agree to indemnify and hold harmless MINTZ PRIVATE INVESTIGATIONS and or owner for errors on databases or documents, in addition, we agree not to indemnify, hold harmless its agents/employees for any and all damages, actions, causes of actions, demands, acts of terrorism either domestic or international, clients misconduct, misinformation/miscalculations, acts of God, subjects elements, acts or factors which are not in control of MINTZ PRIVATE INVESTIGATIONS for now and will not be held liable in the future. Any information we provide you will not be used for any, or all illegal purposes and is punishable by local, state, and federal laws.

INVESTIGATIVE COSTS*

I being the below undersigned agree to pay PRIVATE INVESTIGATIONS a Retainer in the amount indicated below. Our standard investigative rate is $65 per hour, in addition, I agree to a mileage expense of $0.55 per mile and any other expenses unless otherwise discussed and agreed upon. No additional fees or costs will be charged without written and/or verbal authorization. This includes the completed investigation and/or recommendations.As these types of investigations are very sensitive with the current local, state & federal laws we reserve the right to withhold some information. Balance Due; I the below undersigned agree to pay PRIVATE INVESTIGATIONS for any remaining balance due within 10 working days, furthermore I agree to pay 5% compounded every 30 calendar days unless discussed otherwise. We agree PRIVATE INVESTIGATIONS will have the right to retain & not provide privacy information on the subjects companions and agree as the undersigned.

Authorization of Credit Terms*

I have read and agree to these terms and conditions described here in the Credit Card Authorization: We agree to provide the very best service possible. In return, we appreciate and expect your willingness to trust us with your card information. Once you click submit, your request is sent to our office and the Credit Card details are hidden from view. The card information is stored in our secure payment portal (stripe) for your convenience and peace of mind. By Submitting this request you are confirming that you understand that the charges made for services rendered are non-refundable and non-contestable. This authorization agreement shall act as a guarantee of payment for any and all charges incurred for services rendered. We reserve the right to charge your card for any past due balance not paid within our credit terms. Because these transactions are transmitted electronically, it is not required that we keep a physical credit card on file for a charge on the card to be valid. We send out statements on the first of each month for unpaid balances. If payment is not received by the 15th of that same month, we charge the card on file for the total "past due" balance. If you are sending a check too close to the due date, please give us a call and we can discuss alternative payment options.
